Pelvic Floor Therapy and Prolapse

Pelvic organ prolapse (POP) is when one or more pelvic organs descend or bulge into the vagina. This could be the bladder, uterus, rectum or a combination of them. This is one of Dr. Emily’s favorite things to treat, because she has also had prolapse and understands the fear that comes with it.

Up to 50% of women will develop POP at some point in their lives. Symptoms usually consist of a feeling of heaviness or bulging, seeing a lump or bulge, discomfort with sex, problems with urination or defecation, even incontinence.

Prolapse usually occurs for a combination of reasons. One is laxity of the connective tissues or ligaments that support the structure. Pelvic floor weakness could also be a contributing factor, resulting in decreased support of the pelvic organs.

Prolapse can be caused by improper breathing, intra-abdominal pressure problems, pregnancy, labor, weakness, scar tissue, age, connective tissue disorders and straining with defecation.

There are many treatments physical therapy can do to help improve symptoms, and every treatment is individual based on what has caused your prolapse. However, in general we can always start with breathing. If we can help train people to improve their breathing mechanics, we can diminish intra-abdominal pressure and thereby decreasing pressure on the pelvic floor. In addition, we can use body mechanics and strengthening to help improve pelvic organ support.

Pessaries are also another option to help improve prolapse. A pessary is an intra-vaginal support that can be placed by a healthcare professional trained in pessaries to help keep the organs in the correct place.
